A miscalibration of the QAD, or damage taken to the QAD during activation, can cause the creation of dangerous space-time rifts, which can permanent damage the fabric of space, and pose a significant risk to all spacefaring races. Starfleet has not authorized the use of this technology aboard any of it's vessels, however the ''[[USS Venture Star]]'', which the prototype was designed for, still has the unit installed.
